Critically discuss the complex interventions and multidisciplinary team involvement from a holistic perspective in the management of an episode of care – rapid palliative discharge planning from hospital. 1.Choose a service user with complex health and social care needs – complex past-medical hystory, social needs and well-being;2.Critically evaluate pathophysiology and psychological conceptualisations of their complex health conditions (can be multi-organ failure, end-stage Heart Failure, end-stage cancer with multiple metastasis, etc. etc). Can introduce multiple complex health conditions, but need to focus only on one health condition, which will be critically evaluated. This must be highlighted in Introduction.4.Critically review the extent to which the service user, and if appropriate, family/carer perspective was integrated into the coordination of care management. – Involving the family in the process. Emotional/psychological/holistic support of the family. Take in consideration the patient’s wish and personal preferences, such as place to die, DNACPR form, Last Will, spiritual needs, religion, etc. Critically review the breaking of bad news, liaison with the multi-disciplinary team, interpersonal skills of the discharge lead (Palliative Nurse or nurse, that know the family well, such as a district nurse ) in this difficult time for the family and the patient.5.Critically discuss the role and responsibilities of the nurse and other professionals in prioritising, communicating and managing the complexities involved in the service user’s care.
