Mr. Jones is a 95-year old patient with dementia, who is admitted in a long-term care facility. You are the NP caring for Mr. Jones. He recently suffered a stroke, resulting in severe dysphagia. He is being followed by the speech therapist, who communicates that he is at high risk for aspiration, and ideally should be NPO. However, he would then need a feeding tube. According to Mr. Jones advance directive, he is NOT to have a feeding tube under any circumstances. This directive was enacted 10 years ago, when Mr. Jones had no cognitive deficits, and had the full capacity for decision-making. Mr. Jones son is insisting that his father get a feeding tube, because otherwise he would be starved. The facility is concerned about liability.
