You have noted on three separate occasions that the narcotic total does not match what is in the machine. All three occurrences happened soon after the charge nurse and the pharmacy technician filled the Pyxis. What should you do?
Scenario 2: Sandra recently began to question her decision to become a nurse and is considering leaving the profession after only 6 months of licensure. Orientation has left her in tears at least once a week, and she has begun to question her ability to be a safe and competent nurse. Sandra felt belittled by her preceptor and unsupported by her co-workers, who all seemed too busy to help her. She frequently observed her preceptor roll her eyes and sigh loudly when Sandra asked a question. Sandra understood that her preceptor was experiencing conflict in her own life because she commonly overheard her preceptor on the phone arguing with her husband. How will you resolve this dilemma?
Scenario 3: You have just finished assisting with a pulmonary artery catheter insertion on your patient. The physician retorts, This was a tough insertion, and I cannot believe you did not have the defibrillator charged and at the bedside in case we needed it. You are just plain lucky this time.
