Write up a funeral plan that expresses what your wishes would be if you were able to plan your own funeral. Please set up your paper in the following manner:
PART 1: Advance care directives (25% of Grade) (Create your own or download form and fill out and submit)
PART 2: Written Obituary (25% of Grade) (Must write out your own obit)
PART 3: Order of Service & Disposition of Body (25% of Grade) Please describe your final arrangements. Indicate all aspects: Poems; memorial service or funeral; flowers; memorial donations; service details; secular or religious, etc.
Disposition of Body Please indicate disposition of body: embalmed, viewed at home or elsewhere, cremated, buried, entombed, etc.) Also indicate clothing, ashes location, cemetery marker info, etc.
PART 4: Price List (25%): At the end of your write up, please include an itemized price list of all the items included in your funeral with a final cost at the end. Please provide how you will pay for your services.
