Begin by identifying a patient from your clinicals who has a chronic health condition such as diabetes, COPD, hypertension, mental health diagnosis, and post stroke care. Discuss all aspects of healthcare coordination with the chosen diagnosis. Your paper should include the following information: Desсrіption of the patient’s health issues, diagnoses, and treatment plan. Desсrіption of the interdisciplinary team that will participate in this care. Can the CCCR model be used in managing this patient’s care? How so? If not, can you name another model to help in managing this patient’s care? What is the cost of managing this disease? Are there resources available for the patient and family to help with costs and other factors such as commute and etc? What is your role as the APRN in this team? As the APRN, how can you evaluate if the care management is successful? As the APRN, how can you evaluate if this patient needs a higher level of care? Are there any barriers or challenges that you foresee for this patient? How can you overcome these barriers? The body of your paper should be a three to four pages long. This does not include the title page and reference page. Papers that are more than four pages long will not be accepted. Incorporate information from the course text plus a minimum of two (2) scholarly references published within the last five (5) years. Textbooks are not to be considered scholarly references. Use current APA Style to write your paper and to cite your sources. Your source(s) should be integrated into the paragraphs. Use internal citations pointing to evidence in the literature and supporting your ideas. Headings are required. Desсrіption leaves the reader with a complete picture of the setting and patients and diagnoses typically seen. Care Coordination Model Content 20 to ˃18.4 pts Proficient Provides a rich and detailed desсrіption of the care coordination model proposed. Desсrіption is accurate and thorough demonstrating a sophisticated understanding of the care model. Analysis 20 to ˃18.4 pts Proficient Demonstrates a sophisticated understanding and of how the care model will be effective for patients exhibiting similar diagnoses and how the care model will fit into the clinic setting described. Provides an insightful explanation of what will need to change in the clinic setting to improve the fit.Demonstrates a sophisticated understanding of the topic(s) and issue(s). Provides a rich and detailed explanation to the questions.Provides an in-depth, critical analysis appropriately connecting findings to specific clinical concepts, using carefully selected quotes, principles, and/or theories cited from course readings.
